DIRECTIONS FOR REPORTING

Harassment, Intimidation, or Bullying (HIB) Reporting Form 338

 
There are two ways to submit a HIB Form. You can either complete the form online or you can print out a hard copy of the form, complete it, and submit it to any school within the District or you may deposit it into the locked mailbox in front of the Board of Education Office, located at 6028 Broadway, West New York, NJ 07093.  Please address the envelope to: A. Olivero, Assistant Superintendent.

If you cannot use the ONLINE Version, or perhaps do not wish to, you may complete the hard copy/paper form, by clicking here:


To submit the paper form:


    1. Fill out the form: (English) (Spanish) (Arabic)
    2. Print a copy.  If you need a paper copy, but you do not have a printer, please go to your neighborhood school where they will provide you with a printed hard copy of the form. Complete the form and make a copy of it.
    3. Then, the form can be dropped off at any school.  Additionally, you may choose to drop the form off in the locked mailbox which is located in front of the Board of Education Office, located at 6028 Broadway, West New York, NJ 07093.  Please address the envelope to: A. Olivero, Assistant Superintendent.
